Inspect a decision

Open Decisions to investigate an agent request. Filter by agent, action, time, or outcome to narrow the results, then open the decision.

Read the evidence

1. Agent and action: confirm who requested the operation and its arguments.
2. Mode: check Monitoring or Enforcing before interpreting a result of Block.
3. Policy result: read the effect and reason, including whether the agent’s fallback determined it.
4. Request and policy evidence: inspect the captured input, applicable policy versions, matched conditions, and rules.
5. Approval: check whether a reviewer has approved or rejected the request.
6. Timeline and execution reports: compare the authority decision with what the integration says happened.

The decision preserves the policy versions used at the time. Publishing a new policy does not rewrite that evidence.

Investigate unexpected outcomes

If an action was allowed unexpectedly, check assignment, policy scope, rule conditions, and When no policy matches. A policy can apply without any rule matching.

If a blocked action still ran, check the mode and the integration’s execution path. Monitoring intentionally continues. Enforcement only covers calls that pass through the configured check.

If approval is complete but no outcome is recorded, verify that the application resumed the operation and delivered its report. Approval alone is not completion. A missing report leaves the execution outcome unknown.
